PENILE PROSTHETIC SURGERY
Penile prosthetic surgery has been around for many years. Here is a description of a European nobleman's response to the latest prosthetic invention of his time:
His Excellency had recently heard that in the United States, it was possible to be fitted with a curious device; this device was an hydraulic sack that could be fitted within the erectile tissues of the penis. In the abdomen, at the end of the tube, was a reservoir of inert liquid, and in the scrotum was a pump. All that one had to do was squeeze the scrotum in a discreet but vigorous manner, and the fluid would flow from the reservoir to the penis, erecting it magnificently, and all one had to do to detumesce with the dignity of a king was to palpate the pump in the correct fashion. It occurred to his Excellency that theoretically it should be possible to perform sexual alchemy continuously with this device installed. He had already sent off for the brochures, under an assumed name, and the two reasons that he had never got around to having the miraculous operation were that in the first place the pressure of executive business was too great, and secondly, the thought of having an incision made in his penis filled him with irrational hype.
This procedure involves the implantation of an artificial balloon system into the penis. It is placed along the tracts of the corpus cavernosum in the shaft. The modern multi-component inflatable prosthesis is superior to self-contained devices with no differences in complication rates. There are three main types of prostheses.
Malleable or semi-rigid prostheses
This is the least expensive approach and do not have any mechanical parts that can fail. These are two malleable rods are inserted surgically into the shaft of the penis. They produce a permanent erection that is difficult to conceal.
Self-contained inflatable devices
These are the middle of the range. The device provides a controllable natural looking erection. It involves the implantation of a balloon system and reservoir of fluid. The fluid is pumped through and develops a rigid result. This does not, however, add girth to the penis. It is easily concealed.
Multi-component inflatable devices
These are the Rolls Royce of prostheses. They provides a controllable, natural looking erection and may also add girth to the shaft of the penis. When the device is inactivated the penis looks natural in detumescence. This makes it easily concealable. The operation produces a high patient and partner satisfaction.
How long do prostheses last?
Studies have shown that after three years, 85 per cent of these prostheses are still functioning. Once they do begin to develop problems, you would need to have the procedure repeated, or the problem repaired if possible. There is a very high level of satisfaction with these procedures, especially regarding the ability to have intercourse, maintain confidence and fulfilment. Overall, penile prosthesis implantation, although not a first line treatment option, is associated with long-term satisfaction rates.
